Supporting participants workers, therapists and families

Disability Support Workers must complete participant forms and records when supporting the delivery of therapeutic interventions, for any participant on the rising stars program. The information collected supports progress towards NDIS goals, and ensure consistent, person-centred support.

The information collected assists those responsible to monitor outcomes, identify emerging needs, and meet quality and compliance requirements.
